Don Miller | President
Part of the Family Since 1978, 48 Years
Don Miller founded Foursquare Healthcare in 1978 with a vision to provide exceptional care and support for seniors. Through his leadership, dedication, and passion, he has grown the company into the respected organization it is today.
As the Founder, Owner, and CEO, Don’s commitment to excellence has shaped Foursquare’s culture and success. His influence extends beyond the company, having previously served as a state trustee for Texas nursing homes, where he worked to advance standards of care across the state.
Don takes great pride in the Foursquare Family and the lasting impact it continues to make in the lives of residents, families, and staff.
Dan Marick [LNFA] | Director of Operations
Part of the Family Since 2001, 25 Years
Dan Marick has been a vital part of Foursquare Healthcare for 25 years, bringing decades of expertise as a licensed nursing home administrator since 1994. As Director of Operations, he is dedicated to fostering a supportive environment for both residents and staff.
Dan values his role in the Foursquare Family and the relationships it has allowed him to build. Married to his wife LaTisha for over 35 years, he is a proud father and grandfather.
John Miller | Owner & Operations
Part of the Family Since 2000, 26 Years
John Miller’s journey with Foursquare Healthcare began in 2000, working as a rehab technician throughout high school and college. Over the years, his career has grown to reflect his passion for senior care and operational excellence. After managing an assisted living facility in Bridgeport, Texas, for five years, John turned his focus to developing new facilities. Since 2009, he has successfully opened 15 skilled nursing facilities across Texas, expanding Foursquare Healthcare’s reach and impact.
John is proud to carry forward Foursquare’s legacy of growth and innovation, ensuring every facility reflects the company’s core values of care and community.
Heather Burchett [RN, LTC-CIP] | Director of Clinical Services
Part of the Family Since 2007, 19 Years
Heather's journey with Foursquare Healthcare began in 2007 as a brand new LVN, Charge Nurse. Over the past 19 years, she has embraced a variety of roles, including Wound Care Nurse, Assistant Director of Nurses, and Director of Nurses. Today, she serves as our Director of Clinical Services, where she continues her commitment to compassionate care and leadership.
Heather is proud to be part of the Foursquare Family, where she values the strong relationships with both residents and staff.
Timmy Lambe | Director of Physical Plant
Part of the Family Since 2004, 22 Years
For 20 years, Timmy Lambe has been a cornerstone of Foursquare Healthcare’s operations, ensuring the maintenance and functionality of 12 skilled nursing facilities. As Director of Physical Plant, Timmy oversees all aspects of facility upkeep, from routine maintenance to large-scale upgrades, with a focus on safety, efficiency, and reliability. Known for his expertise and resourceful problem-solving, Timmy has a well-earned reputation as the go-to professional for resolving complex challenges. His leadership and dedication play a vital role in providing safe, comfortable environments for residents and staff alike.
Mary Farmer | Compliance Auditor
Part of the Family Since 2002, 24 Years
Mary is a dedicated and knowledgeable team member at Foursquare Healthcare, serving as a go-to resource for many facilities. Hardworking and loyal, she ensures tasks are completed efficiently and is heavily relied upon for support. Her expertise in Foursquare’s compliance program and internal audits is vital to maintaining compliance with Federal, State, and Local regulations, contributing significantly to the Corporate Compliance Committee.
Jeff Patel | IT Director
Part of the Family Since 2001, 25 Years
With over 25 years at Foursquare Healthcare, Jeff Patel has been a driving force behind the technological infrastructure that supports the company’s 12 skilled nursing facilities. As IT Director, Jeff ensures the seamless operation of critical systems, from secure patient record management to the day-to-day functionality of communication networks across multiple locations. Known for his uncanny ability to solve technical issues, Jeff’s expertise has been pivotal in streamlining processes and enhancing efficiency for both staff and residents. Under his leadership, Foursquare Healthcare continues to adopt innovative solutions that improve care delivery and operational excellence.
Jenna Barnes [RN] | Corp Nurse Consultant RN
Part of the Family Since 2011, 15 Years
Jenna joined Foursquare Healthcare in 2011, bringing over 22 years of nursing experience. She began as an LVN Charge Nurse at Senior Care Health and Rehabilitation and has since held several key roles, including Wound Care Nurse, MDS Nurse, Assistant Director of Nursing, and Director of Nursing. Now serving as a Corporate Nurse Consultant, Jenna provides leadership focused on compassionate care and quality service. She takes pride in building strong, lasting relationships with both residents and staff.
Brandy Golden [RN] | Corp Nurse Consultant RN
Part of the Family Since 2017, 9 Years
Brandy began her career with Foursquare in 2017 as a Charge Nurse at Sheridan Medical Lodge. Over the past nine years, she has advanced through several key roles, including Staff Educator and Assistant Director of Nursing (ADON), and currently serves as a Corporate Nurse. In her current role, she specializes in quality measures, quality assurance, and quality improvement initiatives. Brandy is deeply committed to her work and is dedicated to enhancing quality outcomes for residents.
Margaret Shackelford [RN, LTC-CIP] | Corp Nurse Consultant RN
Part of the Family Since 2015, 11 Years
Margaret joined Foursquare in 2015 as the Director of Nursing (DON) at Senior Care Bridgeport. Since then, she has had the privilege of serving the company in various roles, including DON at Cheyenne Medical Lodge and her current position as Corporate Nurse Consultant, which she has held since 2019. With 30 years of nursing experience, Margaret's expertise spans across hospital, hospice, inpatient, and skilled care settings.
Heidi Wefing | Regional, Director of Corporate Compliance
Part of the Family Since 2015, 11 Years
Heidi has over 30 years of experience in long-term care. She has dedicated her career to geriatrics, ensuring that seniors receive the highest level of care they deserve. Heidi is passionate about her residents, their families, and her staff, and she is committed to serving them in any way she can.
Shannon Butler | Director of Physical Plant
Part of the Family Since 2023, 3 Years
Shannon is a husband and father who joined Foursquare in 2023. Originally from Texas, he spent several years in Colorado, where he and his family served as caretakers for Mr. Miller’s property. After returning to Texas to be closer to family, Shannon was invited to join Foursquare. With a passion for serving others, he brings his diverse background and hands-on experience to create a safe and comfortable environment for residents.
Rachel Perez [LVN]| Regional Clinical Reimbursement Specialist
Part of the Family Since 2013, 13 Years
Meet Rachel Perez, our Regional MDS Nurse. With 26 years of nursing experience and 12 years dedicated to our organization, Rachel brings deep clinical knowledge, strong assessment skills, and a passion for quality care. She plays a vital role in ensuring accurate documentation, resident-centered assessments, and smooth coordination across our care teams. Her commitment to excellence and her calm, supportive presence make her an invaluable part of our community.
